Dynamic Balance Measurements Can Differentiate Patients Who Fall From Patients Who Do Not Fall in Patients With Idiopathic Normal Pressure Hydrocephalus

ObjectivesTo investigate which clinical assessments are suitable for differentiating patients who fall from patients who do not fall in patients with idiopathic normal pressure hydrocephalus (iNPH).DesignProspective observational study.SettingOsaka Medical College Hospital.ParticipantsPatients with a cerebrospinal fluid tap test (TT) result meeting the diagnosis criteria for iNPH (N=68).InterventionsNot applicable.Main Outcome MeasuresParticipants were assessed before the TT based on the following: timed Up and Go (TUG), 10-meter walk test (10MWT), Functional Gait Assessment (FGA), Berg Balance Scale (BBS), isometric quadriceps strength (QS), and a history of falls within the past 6 months.ResultsThe full area under the curve (AUC) of the receiver operating characteristic curves and 95% confidence interval were found to be 0.651 (95% confidence interval, 0.503-0.775) for the TUG, 0.692 (95% confidence interval, 0.540-0.812) for the 10MWT, 0.869 (95% confidence interval, 0.761-0.933) for the FGA, and 0.796 (95% confidence interval, 0.663-0.886) for the BBS; except for QS, they all were identified as statistically significant predictive variables. In the TUG<20 seconds group (n=47), the FGA (AUC 0.849 [95% confidence interval, 0.698-0.932]) and BBS (AUC 0.734 [95% confidence interval, 0.550-0.862]) were found to be statistically significant predictive variables; however, the other assessments were not. In the TUG<15 seconds group (n=34), the FGA was found to be the only statistically significant predictive variable (AUC 0.842 [95% confidence interval, 0.640-0.942]), whereas the other assessments were not. The AUC of the FGA was statistically significantly greater than those of the other assessments.ConclusionsOur findings indicate that patients with iNPH who fall experience falls due to dynamic balance dysfunction during gait rather than lower limb muscle strength. The FGA may be more suitable than other assessments for differentiating patients who fall from patients who do not fall in patients with mild iNPH.     

Treatment of burned children using dermal regeneration template with or without negative pressure

ObjectiveEvaluate the results obtained using a Dermal Regeneration Template (DRT) associated or not with the Negative Pressure Wound Therapy (NPWT) for skin cover in paediatric patients who were victims of burns.MethodRetrospective study of a cross-sectional study that evaluated the medical records of children submitted to the application of DRT, associated or not with NPWT, for the treatment of burned children admitted to the Joana de Gusmão Children's Hospital (JGCH) in Florianópolis, in the period from January 2011 to December 2016, totalling 45 patients.ResultsThe majority of the patients were pubescent (31.8%) males (56.8%), and the main aetiology of the burn was flammable substances (75%). In the group that used only DRT, the average take of DRT was 85% with an average time of maturity of 17.65 days and the mean take rate of the skin graft was 85.2%. In the group that used DRT plus NPWT, the mean DRT take was 99.8%, with a mean maturation time of 16.68 days and the mean of the skin graft take rate was 89.1%.ConclusionNPWT associated with DRT offers a higher rate of success in the treatment of complex wounds caused by burns, promotes increased DRT take rate, reduces DRT maturation time and increases the take rate of the skin graft.     

Molecular complexes that direct rhodopsin transport to primary cilia

Rhodopsin is a key molecular constituent of photoreceptor cells, yet understanding of how it regulates photoreceptor membrane trafficking and biogenesis of light-sensing organelles, the rod outer segments (ROS) is only beginning to emerge. Recently identified sequence of well-orchestrated molecular interactions of rhodopsin with the functional networks of Arf and Rab GTPases at multiple stages of intracellular targeting fits well into the complex framework of the biogenesis and maintenance of primary cilia, of which the ROS is one example. This review will discuss the latest progress in dissecting the molecular complexes that coordinate rhodopsin incorporation into ciliary-targeted carriers with the recruitment and activation of membrane tethering complexes and regulators of fusion with the periciliary plasma membrane. In addition to revealing the fundamental principals of ciliary membrane renewal, recent advances also provide molecular insight into the ways by which disruptions of the exquisitely orchestrated interactions lead to cilia dysfunction and result in human retinal dystrophies and syndromic diseases that affect multiple organs, including the eyes.

随着INTERNET网络在国内的普及以及互联网用户的大量增加,以文本信息为载体的网络论坛已经成为人们获取信息,发表个人看法或评论,与其他人进行交流的重要平台。然而,在当前信息爆炸的时代背景下,信息固然重要,但更重要的是对信息内新知识、隐性知识,以及信息发布者潜在需求的深度解读,并基于信息和数据形成相应的思考并提供对策。     

Bardet-Biedl Syndrome: An Atypical Phenotype in Brothers with a Proven BBS1 Mutation

Background: To report the clinical findings in two brothers presenting with a pigmentary retinopathy and post-axial polydactyly, who were found to have a mutation in the BBS1 gene, confirming a diagnosis of Bardet-Biedl syndrome (BBS). Materials and Methods: Documentation of the clinical history, electrophysiological investigations, clinical examination and ocular findings of two brothers born to non-consanguineous white parents, with careful delineation of their clinical phenotypes. Screening of the BBS 1 gene on chromosome 11q13 by PCR-amplified exon alterations followed by direct sequencing was carried out to identify pathogenic mutations. Results: Although both probands had polydactyly and the characteristic ocular signs of BBS on both ophthalmological examination and electro-retinography, neither of them had dysmorphic facial features, obesity, hypogonadism, cognitive impairment, or renal anomalies. The first proband did have mild learning difficulties, although this did not restrict him in activities of daily living. Both probands were homozygous positive for the presence of a c.1169T > G (p.Met390Arg) mutation in BBS1. Conclusion: Although neither proband fulfilled the typical criteria for BBS, this diagnosis was confirmed on mutation analysis. These cases serve to highlight the degree of clinical variability observed in BBS which may be under-diagnosed in patients with milder phenotypes.     

指压夹脊穴治疗肠易激综合征临床观察

目的:评价指压夹脊穴治疗肠易激综合征(IBS)的临床疗效,从中发现致病原理和治疗规律.方法:选取2010至2011年门诊诊治的60例肠易激综合征正在发病的患者,随机分为夹脊穴指压组(指压组)和药物得舒特口服组(药物组)各30例.指压组由专人施行指压夹脊穴治疗,IBS患者腹痛的部位主要为下腹痛,治疗部位主要为第11胸椎、第12胸椎和第1腰椎夹脊穴;药物组采用得舒特(匹维溴铵片)口服治疗.采用肠道症状尺度表(BSS)评分标准进行治疗前后症状评分,应用改善率进行统计,并对统计结果进行分析.结果:两组患者治疗后BSS评分与治疗前比较,差异有统计学意义(均P＜0.01),指压组疗效明显优于药物组(P＜0.01).指压组BSS积分改善优良率90.0％(27/30),明显优于药物组的50.0％(15/30,P＜0.01).结论:夹脊穴指压治疗法可以平衡脊柱阴阳,迅速缓解肠易激综合征症状.肠易激综合征的发病可能与胸腰椎有一定关系.     

中国红丝带网论坛会员信息及邮件订阅系统利用调查分析

目的对中国红丝带网论坛会员的基本信息、关注内容及邮件订阅系统服务（MAIL定制）的利用情况进行调查,探讨改进网络信息交流的方法与策略。方法利用网络论坛注册系统,收集论坛所有注册会员的相关信息,对其基本情况、关注内容及邮件订阅系统利用率进行调查分析。结果在论坛的3029名注册会员中,78.14%为个人求询者,21.86%为艾滋病防治工作者。662名艾滋病防治工作者中82.48%来自非政府组织、政府组织及学院/研究院;46.98%来自于以艾滋病感染者为服务对象的组织。会员的工作领域涉及面比较广;在6个主题分区中,会员对爱之方舟感染者论坛的信息获取需求量最大,其会员数达到1228名,需求率为34.93%。邮件订阅系统的确认接收率为25.72%（779人）。结论网络论坛在对艾滋病感染者这样的边缘化弱势群体进行心理咨询、政策宣传方面仍然起着至关重要的作用;要加强信息平台建设及有效的邮件订阅系统服务,从而更好地进行信息服务的推广。     

试论药品相关论坛(BBS)的管理

介绍了互联网药品相关论坛的发展背景、信息内容要求，并结合我国现行的相关法律法规分析了药品相关论坛当前发展的主要矛盾和症结，提出了加强药品论坛监管体系建设的建议。     

Cerebrospinal fluid cholecystokinin, bombesin and somatostatin in schizophrenia and normals

Cerebrospinal fluid from 31 normals and two groups of phenomenologically similar schizophrenics (n = 72) were collected by identical methods. Radioimmunoassay of CSF was carried out for somatostatin, bombesin, and cholecystokinin. One group of schizophrenics had increased baseline somatostatin and cholecystokinin, and decreased bombesin. No CSF gradient effect was found for the peptides nor were their levels affected by probenecid or pimozide treatment. An inverse correlation was found between bombesin and psychosis rating. Intercorrelation between the peptides and HVA, 5-HIAA, and MHPG were not significant.     

Measurement of antibody and antigen concentrations with a sodium sulphate glutaraldehyde technique

Gamma-globulins can be crosslinked at low concentrations by simultaneous treatment with sodium sulphate and glutaraldehyde (SSG-method). This precipitate can be used for the assay of antibody or antigens. Examples are given for the measurement of (1) antidinitrophenyl antibody by reaction of the precipitate with ¹²⁵I-dinitrophenylovalbumin, (2) IgA by inhibition of the reaction of an anti-IgA precipitate with an ¹²⁵I-labelled IgA myeloma protein, (3) idiotypic antibody by inhibition of the binding of a ¹²⁵I-labelled myeloma protein by an anti-idiotype precipitate.
